Fudging serious, how can you create this?

I cant see how this can be so realistic and great. Its so smooth but the colors bring the best of out of it. This must take a while wouldnt it? Just how could you create something like that?

3d , combined with photoshop, thats pretty much it. would take a while, many layers, it pretty much starts with a concept drawn on paper, and then evolved within photoshop.
