Best Free Form Mail w/autoresponder?

I'm familiar with FormMail (from scriptarchive.com) and AlphaMail but I need the features of both within the same script. FormMail doesn't seem to have auto-respond, and AlphaMail doesn't seem to support unlimited form fields (without editing the script).
Any suggestions on and easy to install, simple, FormMail w/autorespond? I need to have Text input fields, Radio Buttons, and Dropdown lists/menus.

Well, for the record, I found one that is very simple and has some great options. It took many tries to find the right Form Mail script to do what I was looking for and this is what I found to work well:

Good deal. SOrry I couldn't find something sooner. If I am looking for something I usually just figure out how to code it myself. :)
