Upload form by saturday

Hi everyone

I need an upload form created by saturday. It's a short deadline, so just PM me your bid. The deadline on this is definite.

- Check out the screenshot. The form should insert the fields into a record and upload the file.
- We'd also like a record list with edit and delete functions.
- The form does not need the reset button.

The database is already setup and populated with about 10 records. I'll give you the appropriate info. The form below is what we're using now, but the files don't always make it to the server for whatever reason. We're just trying to get this piece solidified.

What type of database is it? Also what language are you asking for? Is it PHP/MySql?

The member who took this job communicated well until the day of the mutually set deadline, and I haven't heard from them since. Which made me miss MY deadline. This job is officially back open to someone who can get this done ASAP.

PHP/SQL btw. Sorry for leaving that out of the original :)
