AS2 email form with attachment

I am flabbergasted that I cannot find a resource for building an email form with attachment capabilities.

I have built a form that calls on PHP to delivery a standard, text-based email, but I really need help building a form with attachments.

Can ANYONE please give me some direction on how to do this?

You can't find anything because you're looking in the wrong place. The thing that writes and sends the email is the thing that puts the attachments in too (as that is part of writing the email.)
