Flash mailform for existing website (PHP)

Hello, I am looking for someone to hire that can build/integrate a custom mailform into my website. I can provide more details if you are interested.

I have been a member of this site for quite some time "back in the day" but but am now using this new username (couldn't remember my password and no longer have the email address that is on file to retrieve it).

I already have a basic one using the Kirupa tutorial but don't have the time to figure out how to build a more advanced one.

Since we are conducting business over the internet I would really prefer someone who can prove they are legit whether by having a website or past references if a deposit to begin is required.... I can send payment via paypal.

This is for a business I am starting and money is limited. However, I don't expect you to work for free either.
