Need Flash based application form create

Hi,

Can anyone re-create this html application form http://www.approvemysite.com/globalhair/application.html to be utilize in Flash MX 2004 (not Pro 2004)?

I need to be able to add a Merchant's script to it when it's completed. If that information helps.

You use php for it, and all it is, is just finding out how to make flash send something from it to your email or something, then just copy and paste stuff around how you like it. But sure, I would be glad to help you.

I need this HTML application form (http://www.approvemysite.com/globalhair/application.html)created in a Flash site. I already have the site and the page which it will reside. The Flash page has a scroller which will accomadate the long application form. The form needs to be able to accept credit card options. I have not received the script from my client's Merchant Account Mgr as of today.
