You need to make a small logo first, 16x16 pixels and then you need to save it as a Windows incon file (.ico extension). Off the top of my head I can't remember if Photoshop supports this, if not use the online tool here to convert it.

Now save the .ico file anywhere on your sever (any directory) and in the HEAD section of your code enter the following:

also, if you want to the icon to to apply to the entire website just put the icon file in your main directory, named favicon.ico
Photoshop does not support .ico so you'll have to use another program, there's a few sufficient ones out there if you search for it.
